Since its first edition held in 1951, athletics events have been competed at the Pan American Games . The events list follows the Olympic model as the Games are an IOC sanctioned competition.

The Pan American Games is a continental multi-sport event in the Americas featuring summer sports, in which thousands of athletes participate in a variety of competitions. The competition is held among athletes from nations of the Americas, every four years in the year before the Summer Olympic Games. It is the second oldest continental games in the world. The only Winter Pan American Games were held in 1990. In 2021, the Junior Pan American Games was held for the first time specifically for young athletes. The Pan American Sports Organization (PASO) is the governing body of the Pan American Games movement, whose structure and actions are defined by the Olympic Charter.

Kim Collins is a former Kittitian track and field sprinter. In 2003, he became the World Champion in the 100 metres. He represented his country at the Summer Olympics on five occasions, from 1996 to 2016, and was the country's first athlete to reach an Olympic final. He competed at ten editions of the World Championships in Athletics, from 1995 to 2015, winning five medals. He was a twice runner-up in the 60 metres at the IAAF World Indoor Championships. At regional level, he was a gold medallist at the Commonwealth Games and a silver medallist at the Pan American Games. As of 2023, he is the only Individual World Championships Gold medallist from Saint Kitts and Nevis.

Panama competed at the 2004 Summer Olympics in Athens, Greece, from 13 to 29 August 2004. This was the nation's fourteenth appearance at the Olympics since its debut in 1920. Panama did not compete on four occasions, including the 1980 Summer Olympics in Moscow, because of its partial support of the United States boycott.

Derrick Ralph Adkins is a former American track and field athlete who specialized in the 400-meter hurdles. He was an Olympic gold medalist in that event at the 1996 Summer Olympics and World Champion at the 1995 World Championships in Athletics. He was the fastest man in the world in the 1994 and 1996 seasons, and holds a personal record of 47.54 seconds. Adkins was a two-time national champion at the USA Outdoor Track and Field Championships.

Lauryn Williams is an American sprinter and bobsledder. She was the gold medalist in the 100 meter dash at the 2005 World Championships in Athletics and won silver medals at the 2004 Summer Olympics, 2007 World Championships, and 2006 IAAF World Indoor Championships. She won a silver medal in the two-woman bobsleigh at the 2014 Winter Olympics.

Bernard Rollen Williams III is an American male former track and field sprinter and winner of a gold medal in 4 × 100-meter relay at the 2000 Summer Olympics. He was the 200-meter dash silver medalist at the 2004 Summer Olympics and the 100-meter dash silver medalist at the 2001 World Championships in Athletics. He also won relay gold at the 2003 World Championships in Athletics and was the 100 m gold medalist at the 1999 Pan American Games.

Adhemar Ferreira da Silva was a Brazilian triple jumper. He won two Olympic gold medals and set four world records, the last being 16.56 metres in 1955 Pan American Games. In his early career he also competed in the long jump, placing fourth at the 1951 Pan American Games. He broke world records in triple jump on five occasions during his illustrious career. To date, he remains the only track and field athlete from South America to have won two Olympic gold medals.

The Men's Pole Vault event at the 2003 Pan American Games took place on Friday August 8, 2003. Bronze medalist Dominic Johnson won the only medal for Saint Lucia at the 2003 Pan American Games.

Randy Jenelle Givens is an American former track and field athlete who competed in sprinting events. Givens was a 200 m finalist at the 1984 Olympic Games and represented her country at the 1983 World Championships in Athletics. She was the 200 m gold medalist at the 1983 Pan American Games and returned to win a silver medal at the 1987 edition.
