Mayday , known as Air Crash Investigation(s) outside of the United States and Canada and also known as Mayday: Air Disaster (The Weather Channel) or Air Disasters (Smithsonian Channel) in the United States, is a Canadian documentary television series produced by Cineflix that recounts air crashes, near-crashes, fires, hijackings, bombings, and other mainly flight-related disasters and crises. It reveals the events that led to each crisis or disaster, their causes as determined by the official investigating body or bodies, and the measures they recommended to prevent a similar incident from happening again. The programs use re-enactments, interviews, eyewitness testimony, computer-generated imagery, cockpit voice recordings, and official reports to reconstruct the sequences of events.
As of 12 December 2023, [update] 270 episodes of Mayday have aired. [n 1] This includes five Science of Disaster specials, each examining multiple crashes with similar causes. For broadcasters that do not use the series name Mayday, three Season 3 episodes were labelled as Crash Scene Investigation spin-offs, examining marine or rail disasters.
A sub-series labelled The Accident Files begain airing in 2018 and, as of 2023, has aired five seasons consisting of ten episodes per series. This sub-series consists entirely of summarized versions of air disasters previously investigated in the primary Mayday series, but combined based on similarities between the incidents, such as fires or pilot error. Each episode covers three accidents and 15 minutes is dedicated to each of the disasters that are covered.

The Boeing 757 crashes into the Pacific Ocean, killing everyone on board. The false readings and contradictory warnings were caused by duct tape over the static ports, which was used to protect the ports during maintenance, but was not removed afterwards. On 24 December 1994, Air France Flight 8969 is hijacked on the ground at Algiers Airport. The four terrorists demand the aircraft be allowed to depart for Paris so they can blow up the aircraft over the Eiffel Tower, but the Algerian Army blocks the aircraft with vehicles and refuse to allow it to take off. Unable to carry out their plans, the terrorists kill three passengers over the next two days. The aircraft is then cleared to take off, but the crew diverts to Marseille Provence Airport. The hostage rescue team of the French Gendarmerie storms the aircraft, and after a gun battle in the cabin, the hijackers are killed, while everyone else survives. On 1 July 2002, BAL Bashkirian Airlines Flight 2937 collides with DHL International Aviation ME Flight 611 in German airspace near Überlingen, killing all 71 people on board the two aircraft. The crew of Flight 611 followed the on-board traffic collision avoidance system (TCAS) instructions to initiate a descent. However, the crew of Flight 2937 followed the instructions of the air traffic controller instead of their TCAS and initiated a descent as well. Only one air traffic controller was on duty (covering two boards) for that block of airspace at the time of the collision—his co-worker was on his coffee break. The pilots accept the offer, but as they are making the necessary changes, they inadvertently delete the waypoints from the flight plan in their flight management system, causing them to lose certainty of their exact position. On 25 January 1990, Avianca Flight 052 is delayed numerous times by bad weather en route and is dangerously low on fuel as it attempts a landing at New York City. Wind shear forces the crew to abort the approach just one mile from the runway. They initiate a go-around, but are directed back into a holding pattern by air traffic controllers who are unaware of the low fuel situation. On 12 May 1989, a 69-car freight train goes out of control while descending from California's Cajon Pass. It derails in a residential neighbourhood of San Bernardino after reaching speeds in excess of 100 miles per hour (160 km/h). Two residents and two train crew members are killed in the initial crash. More than a week later, an underground gasoline pipeline, damaged by earth-moving equipment during the post-crash clean-up, ruptures and sparks a fire that kills another two people. Clerks in Mojave had greatly underestimated the train's weight, and it had been assembled without enough locomotives to provide adequate braking. On 9 June 1996, Eastwind Airlines Flight 517 also rolls unexpectedly in similar circumstances, but the crew successfully regains control of the aircraft and lands safely. The cause of all three incidents was a design flaw with the rudder's control system which allowed the rudder to suddenly and unexpectedly go to full deflection and jam due to thermal shock of the hydraulic control valve. Nearly two years later, on 3 March 1974, Turkish Airlines Flight 981 crashes near Senlis after suffering a similar sequence of events during a flight from Paris to London, killing all 346 people on board. A design flaw with the DC-10's cargo door locking mechanism was not rectified after the first accident, and the second DC-10's door opened during flight, causing the crash. The Linate Airport disaster occurred in Italy at Linate Airport in Milan on the morning of Monday, 8 October 2001. Scandinavian Airlines System Flight 686, a McDonnell Douglas MD-87 airliner carrying 110 people bound for Copenhagen, Denmark, collided on take-off with a Cessna Citation CJ2 business jet carrying four people bound for Paris, France. All 114 people on both aircraft were killed, as well as four people on the ground.
Aeroperú Flight 603 (PL603/PLI603) was a scheduled passenger flight from Miami International Airport in Miami, Florida, to Arturo Merino Benítez International Airport in Santiago, Chile, with stopovers in Quito, Ecuador, and Lima, Peru. On October 2, 1996, the Boeing 757-23A aircraft flying the final leg of the flight crashed, killing all 70 people aboard.
Aloha Airlines Flight 243 was a scheduled Aloha Airlines flight between Hilo and Honolulu in Hawaii. On April 28, 1988, a Boeing 737-297 serving the flight suffered extensive damage after an explosive decompression in flight, caused by part of the fuselage breaking due to poor maintenance and metal fatigue. The plane was able to land safely at Kahului Airport on Maui. The one fatality, flight attendant Clarabelle "C.B." Lansing, was ejected from the airplane. Another 65 passengers and crew were injured. The substantial damage inflicted by the decompression, the loss of one cabin crew member, and the safe landing of the aircraft established the incident as a significant event in the history of aviation, with far-reaching effects on aviation safety policies and procedures.
Arrow Air Flight 1285R was an international charter flight carrying U.S. Army personnel from Cairo, Egypt, to their home base in Fort Campbell, Kentucky, via Cologne, West Germany, and Gander, Newfoundland. On the morning of Thursday, 12 December 1985, shortly after takeoff from Gander International Airport en route to Fort Campbell, the McDonnell Douglas DC-8 serving the flight stalled, crashed, and burned about half a mile from the runway, killing all 248 passengers and 8 crew members on board. As of 2024, it is the deadliest aviation accident to occur on Canadian soil. At the time of the crash, it was the deadliest aviation accident involving a DC-8; as of 2024, it is the second-deadliest, behind the crash of Nigeria Airways Flight 2120 nearly six years later.
Air Transat Flight 236 was a transatlantic flight bound for Lisbon, Portugal, from Toronto, Canada, that lost all engine power while flying over the Atlantic Ocean on August 24, 2001. The Airbus A330 ran out of fuel because of a fuel leak caused by improper maintenance. Captain Robert Piché, 48, an experienced glider pilot, and First Officer Dirk DeJager, 28, glided the plane to a successful emergency landing in the Azores, saving all 306 people on board. Most of the passengers on the flight were Canadians visiting Europe or Portuguese expatriates returning to visit family in Portugal. This was also the longest passenger aircraft glide without engines, gliding for nearly 75 miles. Following this unusual aviation accident, this aircraft was nicknamed the "Azores Glider".
United Airlines Flight 811 was a regularly scheduled airline flight from Los Angeles to Sydney, with intermediate stops at Honolulu and Auckland. On February 24, 1989, the Boeing 747-122 serving the flight experienced a cargo-door failure in flight shortly after leaving Honolulu. The resulting explosive decompression blew out several rows of seats, killing nine passengers. The aircraft returned to Honolulu and landed without further incident.

USAir Flight 1016 was a regularly scheduled flight in the southeastern United States, between Columbia, South Carolina, and Charlotte, North Carolina. On July 2, 1994, the flight encountered heavy thunderstorms and microburst-induced windshear while attempting to land, and crashed into heavy trees and a private residence near the airport. The crash and ensuing fire caused 37 fatalities and seriously injured twenty others.
Aeroflot Flight 593 was a passenger flight from Sheremetyevo International Airport, Moscow, Russia, to Kai Tak Airport in Hong Kong. On 23 March 1994, the aircraft operating the route, an Airbus A310-304 flown by Aeroflot, crashed into the Kuznetsk Alatau mountain range in Kemerovo Oblast, killing all 63 passengers and 12 crew members on board.
South African Airways Flight 201 (SA201), a de Havilland Comet 1, took off at 18:32 UTC on 8 April 1954 from Ciampino Airport in Rome, Italy, en route to Cairo, Egypt, on the second stage of its flight from London, England to Johannesburg, South Africa. The flight crashed at around 19:07 UTC, killing all on board. The flight was operated as a charter by British Overseas Airways Corporation (BOAC) using the aircraft G-ALYY, with a South African crew of seven, and carrying fourteen passengers.

United Airlines Flight 173 was a scheduled flight from John F. Kennedy International Airport in New York City to Portland International Airport in Portland, Oregon, with a scheduled stop in Denver, Colorado. On December 28, 1978, the aircraft flying this route ran out of fuel while troubleshooting a landing gear problem and crashed in a suburban Portland neighborhood near NE 157th Avenue and East Burnside Street, killing 10 people on board.

On Thursday, September 8, 1994, the Boeing 737 flying this route crashed in Hopewell Township, Pennsylvania while approaching Runway 28R at Pittsburgh, which was USAir's largest hub at the time.
United Express Flight 6291 was a regularly scheduled United Express flight from Dulles International Airport near Washington, D.C. to Port Columbus International Airport in Columbus, Ohio. It was a service operated by Atlantic Coast Airlines on behalf of United Express.
First Air Flight 6560 was a domestic charter flight that crashed on landing at Resolute, Nunavut, Canada, on 20 August 2011. Of the 15 people on board, 12 were killed and the remaining three were severely injured. The Boeing 737-200 of First Air was operating a service from Yellowknife, Northwest Territories, when it struck a hill in cloud near Resolute Bay Airport.
Northwest Airlines Flight 85 was a scheduled international passenger flight from Detroit Metropolitan Wayne County Airport in the United States to Narita International Airport in Japan. On October 9, 2002, while over the Bering Sea, the Boeing 747-400 experienced a lower rudder hardover event, which occurs when an aircraft's rudder deflects to its travel limit without crew input. The 747's hardover gave full left lower rudder, requiring the pilots to use full right upper rudder and right aileron to maintain attitude and course.
